RHINOPLASTY SURGERY - "NOSE JOB"

When you look at someone’s face, first thing you notice is the nose, the most prominent part on the face. It is done either consciously or unconsciously.


Rhinoplasty surgery is usually done on persons between age 18 and 25.  Both men and women are equally operated on.  Usually this type of surgery is not recommended before 18 years of age because before puberty nose growth is not finished yet. If your health is generally good, there is no upper age limit for this type of surgery.

 

Nose surgery sometimes can be performed in combination with face lift or other rejuvenation procedures in order to correct ageing process of the nose, such as lowering of the tip of the nose. Also breathing problems that are related to nose septum deviation can be solved at the same time as aesthetic nose shaping is completed

.

NOSE SURGERY CANDIDATES

Indications that show that you are a candidate for rhinoplasty are: that your nose is too big for your face, that kyphosis is present, i.e. that you have long nose or humped nose, that you have a wide tip of the nose or wide alar base, that your nose is wide or it is like a saddle, lowered or raised tip of the nose or your nose is asymmetric and deviated.


Before the surgery, you must have a clear idea of how you would like your nose to look and, at the same time, to have realistic understanding of limitations of the procedure. If you are interested in this type of surgery, Prof. Dr. Milan Jovanovic can give you further information.


PREOPERATIVE ASSESSMENT

Patient preparation for nose surgery usually starts during the first examination. The first step is the patient consent. Very important part is to create that feeling of trust in the doctor-patient relationship. The patient will be informed of surgery process and what surgery includes. During the initial examination you’ll be asked to look in the mirror and show precisely what kind of change you would like to be done to your nose.


During the examination you will give data of your medical history of illnesses or surgical treatments you had previously, nose injuries or medicine allergies, chronic infections, hypertension, diabetes, anemia. It is very important to provide your doctor with complete information. After discussion of medical history, an examination of inner nasal structures should be done, as well as examination and quality of nose skin, size and shape, and relation of the nose with other structures of the face.


Advices that patient need to follow are: two weeks before nose surgery do not use medicine such as aspirin, estrogen, vitamin E and C. Do not make plan to undergo surgery in the middle of menstrual cycle. Do not eat or drink eight hours before the surgery.


RHINOPLASTY PROCEDURE

Nose surgery can be performed in general endotracheal anesthesia (that allows higher comfort to the patient and surgeon) as well as in local anesthesia (which requires special technique) and intravenous analgosedation. Throughout the surgery, blood pressure, heart function, pulse and oxygen levels in your blood are monitored.


POSTOPERATIVE TREATMENT

After nose surgery a splint will be placed on the nose ridge in order to immobilize nose tissue. After the surgery you will be moved to unit of semi-intensive care where you will still be postoperatively monitored for some time. After a few hours you can be discharged from hospital or you can stay for a night in hospital if the surgeon or the anesthesiologist decides.


In general, bruises around cheeks and eyes are most noticeable during the first five days after surgery. If needed, make-up can be used to cover the bruises. Significant swelling can last up to several weeks. Minor residual swelling, usually around the tip of the nose can stay there up to several months. Usually it is not noticeable and other people do not notice it. If the immobilization is used this will disappear within three to seven days after the surgery. Sutures are removed after 7 to 14 days after the surgery is complete. If lateral and medial osteotomy is done, nasal splint may be left for several weeks postoperatively.


COMPLICATIONS

Significant complications by rhinoplasty occur rarely. Every year in the world several thousands of people undergo rhinoplasty and in general, if the surgery is performed correctly, all of the patients are pleased with the results. Despite that, anyone who decides to go through this type of surgery must understand that every surgery has its own risks.


Possible complications that may be considered includes the appearance of hematoma, reactions to anesthesia, infections or skin numbness that is usually temporary. Increased bleeding can be caused by aspirin or some of the anti-inflammatory drugs, so all of these drugs should not be used after the surgery. Some plastic surgeons recommend not smoking after the surgery.


During early postoperative course patients should avoid tensing, heavy lifting, bending. After seven to ten days after surgical treatment you can continue normal activities. Exposing to the direct sunlight should be avoided for a few months, because the nose may be sensitive during this time. Protective creams should be used. Avoid wearing heavy glasses for the first three weeks, if changes to your nose bones were done.


Aim of the rhinoplasty surgery is that the nose gets more natural look and that it harmoniously fits shape of your face. Healing after nose surgery is very long. Patient should wait up to one year to see the final results of the rhinoplasty. However, first results can be seen already after a few weeks. Mostly outcomes of rhinoplasty are permanent, with the exception of changes related to normal ageing process.
